Step 1

Intake form

The intake form helps me understand a little about you and what's bringing you to counselling. It covers your contact details, an emergency contact, and some questions about how you've been feeling.

Answer as much as you feel comfortable with. If you'd rather talk about something in person, you can leave it blank.

Your answers are sent securely and only Shiyanka can read them. You'll get an email confirming we've received your form.

Step 2

Confidentiality agreement

What you share in counselling is kept confidential as far as possible. There are some situations where confidentiality must be waived and the appropriate authorities or family members notified, because someone is at risk:

  • if you are at risk of serious harm or are considering suicide
  • if you are at risk of harming another person
  • if there is abuse of a child, which I'm required by law to report

On occasion, counselling notes may also be subpoenaed by a court order.

Where confidentiality can't be maintained, I'll take every possible step to discuss it with you first.

You'll sign this agreement online, with your finger or mouse, at the end of the intake form.

Step 3

Cancellation policy

  • Please give at least 48 hours' notice if you can't attend your session.
  • If you cancel with less than 24 hours' notice, a fee of 50% of the session cost applies.
  • If an appointment is missed, the full session fee applies.
  • Any fees need to be paid before your next session.

Why this matters

This policy gives everyone a fair chance to access support. With 48 hours' notice, I can offer your time to someone who's waiting for a session.

Thank you for your understanding.

You'll sign this policy online at the end of the intake form, and a copy is emailed to you.

Ready when you are.

Once you've booked, your intake form is all you need before we meet.
